im wondering if there is a neater way for rain to go instead of falling onto the tiles and then into a lower gutter

They can have gutters. If it is a flat roof dormer, and discharges rainwater at the front edge where the window is, usually a gutter is fitted to the dormer with a "shoe" at the bottom of the downpipe so that the water goes onto the house roof and then runs down to the main house gutter. (it would look very ugly if the downpipe was extended right down the house roof into the main gutter) I used greenhouse guttering/downpipe on my dormer, it is adequate to cope with the amount of rainwater from the size of dormer I have and it's size is more proportionate.

some larger dormers will have side gutters that discharge back on the main roof area. It would be even more untidy to have a down pipe from the dormer.

Yes many dormers have gutters how ever you can have words with some experts like http://www.asggutter.com/ for a better understanding

depending on the direction of your roof, yes, many dormers have guters. Having tiles maybe more difficult, as I am more acustom to aspault shingles

yes, they should have gutters running backwards to the roofline so the water drips onto flashing or tiles/slates.
